热敏灸对膝骨性关节炎兔模型炎症因子的影响

摘要:研究目的:观察热敏灸对KOA兔模型白细胞介素-1(Interleukin-1β,IL-1β)、肿瘤坏死因子-α(Tumor necrosis factor-α,TNF-α)、基质金属蛋白酶-13(Matrix Metalloproteinase-13,MMP-13)的影响,探讨热敏灸治疗KOA的作用机制。研究方法: 42只雄性日本长耳兔按随机数字表随机分为空白组(6只)、模型组(6只)、艾灸组(24只)、假手术组(6只)。空白组正常饲养,模型组与艾灸组采用木瓜蛋白酶注射膝关节建立KOA模型,假手术组采用腔内注射生理盐水造模,每日强迫活动30min,造模15天。造模结束后,艾灸组艾灸犊鼻穴,每日1次,每次40min,共14天,根据家兔艾灸过程中温度变化分为热敏灸组和非热敏灸组,两组各随机抽取6只;空白组、模型组、假手术组正常饲养,不予任何干刺激。治疗结束后,采用放射免疫法检测关节液IL-1β、TNF-α含量,Real-time PCR检测法检测软骨MMP-13含量。研究结果:与空白组相比,模型组、热敏灸组、非热敏灸组、假手术组IL-1β、TNF-α、MMP-13表达均升高,差异均明显(P<0.05,P<0.01);与模型组相比,热敏灸组、非热敏灸组、假手术组表达均明显偏低(P<0.05,P<0.01);与非热敏灸组相比,热敏灸组表达明显偏低(P<0.05,P<0.01)。结论:热敏灸能抑制IL-1β、TNF-α、MMP-13表达,减少炎症反应,缓解关节损害,这可能是热敏治疗KOA的效应机制之一。

Effect of Heat-sensitive Moxibustion on inflammatory factor in Rabbits with Knee Osteoarthritis

Abstract: Objective: To observe the effects of heat-sensitive moxibustion on the content of Interleukin-1β(IL-1β)、Tumor necrosis factor-α(TNF-α)and Matrix Metalloproteinase-13(MMP-13)in KOA rabbits, and discuss the mechanism of heat-sensitive moxibustion in treating KOA. Methods: 42 male rabbits were randomly divided into four groups: blank control group(n=6), model group(n=6), moxibustion group(n=24) and sham-operated group(n=6). Blank control group was fed normally, model group and moxibustion group were injected with papain in order to set up KOA model, and sham-operated group was injected with normal saline. From the fisrt day of modeling, all rabbits(except blank group) were forced to move 15 days, every day for 30min. After modeling, moxibustion group treated with suspended moxibustion at the point Dubi(ST35) ,40 min once time, once per day, totally 15 days. Check and record the changes of rabbits’ temperature every 10min to divide the moxibustion group into heat-sensitive moxibustion group and non heat-sensitive moxibustion group. Two groups were randomly selected 6 rabits. The other three groups were fed as usual, with no special treatment. At the end of treatment, the content of IL-1β and TNF-α in joint fluid was examined by radioimmunoassay, and the content of MMP-13 were examined by Real-time PCR. Results: The level of IL-1β、TNF-α and MMP-13 of other groups were significantly lower than blank control group(P<0.05,P<0.01);  compared with model group, moxibustion group and sham-operated group were significantly lower(P<0.05,P<0.01);and the levels reduced significantly compared heat-sensitive moxibustion group with non heat-sensitive moxibustion group(P<0.05,P<0.01). Conclusion: Heat-sensitive moxibustion could treat KOA and reduce joint damage, which might be through inhibiting the content of IL-1β、TNF-α、MMP-13 and reducing inflammation.
